- You will need to add CBNC as your authorized agent:
- Under ‘Authorized Agent Search Type’, check ‘Authorized Agent ID or Name Search’
- Under ‘Authorized Agent ID or Name’, enter ‘Chicago Bilingual Nurse Consortium’
- Check the box starting with ‘I authorize CGFNS…’
- You will want to add Neyda Mucha as your authorized representative:
